Frequently Asked Questions

1. What is the DCW used for?

The Data Collection Worksheet (DCW) provides the Bureau of Health Workforce (BHW) with school information related to a student's tuition costs, school incurred fees, and student expenses. The NURSE Corps Scholarship Program and National Health Service Corps Scholarship Program use this information to accurately calculate the scholarship amount for each participant. Please note: Student expenses are costs incurred by the student and BHW will be responsible for reimbursing these students and not the school.

2. I am unable to log into the School Representative Portal

Please scroll down to the bottom of the log-in page and select, Create An Account. Once you have created an account, you will receive an email with a link to activate the account. Follow the instructions in that email and you will then be able to log-in to the portal.

3. I created an account in the School Representative portal but I received an error message that 'the email address is not found in our in the database'

Please call the HRSA Help Desk at 1-800-221-9393 (TTY for hearing impaired: 1-877-897-9910), Monday through Friday (except Federal holidays), 8:00 am to 8:00 pm ET or Contact Us. Make sure you provide your name, title, department, school name and address, email and phone number so that we can add you into our system as a point of contact.

4. I forgot my username

If you have forgotten the email you signed up with or your email address has changed, you will need to contact the HRSA Help Desk at 1-800-221-9393 (TTY for hearing impaired: 1-877-897-9910), Monday through Friday (except Federal holidays), 8:00 am to 8:00 pm ET or Contact Us.

5. I forgot my password

Please go to "I forgot my password" and enter your email. You will be required to answer the security question you selected when creating your account. You will receive an email with a link to reset your password. 6. What do I do if my account is locked?

Your account will be locked after 3 failed attempts to log in. If this happens go to "I forgot my password" link and follow the instructions from the "I forgot my password" FAQ above.

7. Where can I locate my school's DUNS number?

Please go to sam.gov and click on Search Records to perform a search on your school’s name. If your school does not have a DUNS number, please go to dnb.com/get-a-duns-number.html to register.

8. Do I need to complete a DCW for each student applying to the National Health Service Corps Scholarship Program and NURSE Corps Scholarship Program?

Each scholarship program requires only one DCW per school and per degree; not per student. However, if that student has applied to both scholarship programs, you will need to fill out a DCW for each program as they are two different programs. If your institution offers multiple programs/specialties, please submit the DCW which provides the most comprehensive tuition and fee schedule coverage.

9. Should I enter the tuition for one semester only?

Please enter the tuition amount for an entire ACADEMIC year. If your institution uses a semester system, please include the tuition amount to be enrolled as a full-time student for all semesters during the academic year. If your institution uses a quarter system, please include the tuition amount to be enrolled as a full-time student for a fall, winter, spring, and summer quarter. To properly budget for scholarship awards, the Bureau of Health Workforce (BHW) requires information for each academic year of your schools program.

10. Some of the required program fees are not listed on the DCW, are they eligible?

As institutions may have varying names for the types of fees on the form, the Bureau of Health Workforce (BHW) only lists the eligible fees are listed on the DCW Forms. If a required fee at your institution does not fit into any of the fee categories noted, please list that fee in the 'Other' category. A comment explaining the fee will be required when populating the “other” category.

* Please note that 'Other' fees may not be eligible for reimbursement due to fee restrictions.

11. How much should I enter for Other Reasonable Costs (ORCs)?

Please enter your programs estimates for each ORC type for an entire ACADEMIC year. ORCs are different between the two scholarship programs so if the applicant is applying to both programs you will need to complete a DCW for each program.

12. Can I edit a DCW that I already have submitted?

Yes, as long as the Program deadline for that DCW has not passed. If the deadline has passed and you need to make changes to your form please contact either the NURSE Corps Scholarship Program or the National Health Service Corps by email at NURSE_Corps_SP@hrsa.gov or nhscsp@hrsa.gov in order to make these changes.

13. May I submit a paper copy of the DCW instead?

Please use the School Representative portal to complete the DCW. If you experience technical difficulties, please contact the NURSE Corps Scholarship Program or National Health Service Corps Scholarship Program by email at NURSE_Corps_SP@hrsa.gov or nhscsp@hrsa.gov for assistance. If we are unable to resolve the issue, an alternative method of submission may be offered.
